Report Mandatory Occurrence

Objective

The objective of this process is to collate service performance information and in addition meet the regulatory obligations of the state.

Scope

The scope of this process covers all occurrences that are defined by the organisation as requiring mandatory reporting. The list of occurrences will be a list of events defined by the organisation through the safety risk assessment process (defined as performance indicators) as well as those defined by the state regulatory authority.

Trigger

This process is triggered by any employee who participates in or observes a safety occurrence that is described as a mandatory reportable occurrence by the organisation.
